Moderna, Merck’s Cancer Vaccine Breakthrough Raises Hope For New Wave Of Personalized Treatments
A personalized cancer vaccine developed by Moderna and Merck has delivered a major breakthrough in melanoma, raising hopes that the same approach could eventually be used against a much wider range of cancers.
The treatment, known as intismeran autogene, is designed individually for each patient using mutations found in their tumor. Combined with Merck’s immunotherapy drug Keytruda, it significantly reduced the risk of melanoma returning or spreading in a Phase 3 study of 1,137 patients whose tumors had been surgically removed but who remained at high risk of recurrence. The companies said the study met its main goals of improving recurrence-free survival and distant metastasis-free survival, with no new safety concerns identified.
The results are being closely watched because they could validate a new way of treating cancer. Instead of attacking rapidly dividing cells like chemotherapy or broadly stimulating the immune system, personalized cancer vaccines are designed to teach a patient’s immune system to recognize mutations specific to their cancer and target cells carrying them.
The next question is whether the approach can deliver similar results against other tumors. Moderna and Merck already have large studies underway involving patients with non-small cell lung cancer whose tumors have been surgically removed, while mid-stage studies are testing the combination in bladder and kidney cancers. Early-stage studies are also examining its potential in pancreatic and stomach cancers. Moderna President Stephen Hoge told Reuters that results from numerous trials are expected over the next year or two.
The companies are not the only ones pursuing such treatments. BioNTech and Roche are developing a rival individualized mRNA treatment called autogene cevumeran, which is being tested in patients with colorectal and pancreatic cancers following surgery. BioNTech says the treatment is designed to generate an immune response against cancer mutations unique to each patient’s tumor.
The Moderna-Merck vaccine follows a similar principle but requires a highly individualized manufacturing process. A sample of a patient’s tumor is analyzed to identify mutations that could be recognized by the immune system, and those findings are then used to manufacture a customized mRNA vaccine. Patients in the melanoma trial received up to nine doses of the vaccine alongside up to nine doses of Keytruda, given every six weeks, CNBC reported.
Earlier results had already provided evidence that the approach could have lasting benefits. Five-year follow-up data from a Phase 2b trial showed that the vaccine-Keytruda combination reduced the risk of cancer recurrence or death by 49% and the risk of distant metastasis or death by 59% compared with Keytruda alone.
The Phase 3 results strengthen that evidence, but important questions remain. Moderna and Merck have so far announced top-line findings rather than the full trial data, and the study is continuing as researchers track whether the treatment ultimately extends patients’ overall survival. Analysts are also waiting for more detailed information before assessing the vaccine’s full clinical and commercial potential.
Another important factor will be whether the treatment’s benefits justify adding a personalized vaccine to existing immunotherapy. Keytruda can cause side effects including fatigue, diarrhea, skin reactions and inflammation, but Moderna and Merck said the Phase 3 study identified no new safety concerns from adding intismeran. In the earlier study, reactions associated with the vaccine were generally similar to those seen with conventional vaccinations, Hoge told Reuters.
The breakthrough has also revived investor interest in mRNA technology beyond infectious diseases. Moderna’s market value jumped by roughly $30 billion following the trial announcement, while other biotechnology stocks also rallied as investors bet that the melanoma success could improve the prospects of other mRNA cancer programs, BBC reported.
