Is Xylitol Bad for You? New Study Links Popular Sweetener to Heart Attack and Stroke Risk


A popular sugar substitute found in sugar-free gum, foods and oral-care products is drawing fresh scrutiny after researchers linked higher blood levels of xylitol to a greater risk of heart attacks, strokes and cardiovascular death.

The findings, presented Saturday, Aug. 29, at the European Society of Cardiology’s 2026 congress in Munich, Germany, come from an analysis of 17,710 people and add to earlier research suggesting that xylitol may make blood platelets more prone to clotting.

Importantly, the new research is observational, meaning it found an association and does not establish that eating xylitol directly causes heart attacks or strokes.

What the New Xylitol Study Found

Researchers examined participants from two large prospective studies: the Canadian Longitudinal Study on Aging and the European Prospective Investigation into Cancer and Nutrition-Norfolk study.

In the Canadian cohort, people with the highest blood xylitol levels had a 57% higher risk of a major adverse cardiovascular event over six years than those with the lowest levels after researchers adjusted for established risk factors including age, sex, smoking, diabetes, hypertension and cholesterol levels.

In the EPIC-Norfolk group, the highest xylitol levels were associated with an 18% higher cardiovascular-event risk over 30 years.

Researchers also observed a dose-response pattern, with cardiovascular event rates rising as blood xylitol levels increased.

Major adverse cardiovascular events included death, heart attack and stroke.

The results build on research published in 2024.

That earlier work found people with the highest xylitol levels were about 50% more likely to experience cardiovascular events over three years than those with the lowest levels.

Laboratory experiments also found that xylitol increased platelet responsiveness and accelerated clot formation.

Why Could Xylitol Affect Heart Health?

One concern involves blood clotting.

Platelets are blood components that help stop bleeding by forming clots.

NIH researchers reported that exposing human platelets to xylitol made them more sensitive to signals that trigger clotting.

In mice, higher xylitol levels were associated with faster clot formation and artery blockage.

In a small human experiment involving 10 healthy volunteers, drinking a xylitol-sweetened beverage caused blood xylitol levels to rise about 1,000-fold within 30 minutes.

Levels returned toward baseline within four to six hours, while platelet sensitivity to clotting signals increased when xylitol levels were high.

Dr. Marco Witkowski of Charité University Hospital Berlin, who presented the new findings, said previous research showed that “xylitol can enhance the ability of platelets to form clots.”

He said the latest results highlight how much remains unknown about the sweetener’s long-term cardiovascular safety.

Does This Mean You Should Stop Eating Xylitol?

Not necessarily. The researchers have not shown that xylitol causes heart attacks or strokes, and the latest study does not establish a specific amount that is dangerous for people to consume.

Professor Dan Atar, a member of the ESC Communication Committee, emphasized that “causalities are difficult to infer” from observational research and said further studies are needed.

The distinction matters because xylitol is a sugar alcohol rather than a high-intensity sweetener such as aspartame or sucralose.

The U.S. Food and Drug Administration lists xylitol among sugar alcohols permitted for use as sugar substitutes, including in products such as sugar-free candies, cookies and chewing gum.

So the new research is not a recall or a finding that xylitol-containing products have suddenly been declared unsafe.

Instead, it raises a longer-term cardiovascular safety question that researchers say needs additional investigation.

Where Xylitol Is Found

Xylitol occurs naturally in very small quantities, but manufacturers can add much larger amounts to products as a sugar substitute.

It can be found in sugar-free chewing gum, candies, certain foods and beverages, and oral-care products such as toothpaste.

For consumers who want to limit their exposure, checking ingredient lists is the most straightforward step.

Xylitol is typically listed by name among the ingredients.

However, experts caution against interpreting the findings as a reason to simply replace xylitol with more added sugar.

Dell Stanford, a senior dietitian at the British Heart Foundation, said the research does not provide “a green light to have more sugar instead,” emphasizing the broader importance of reducing highly processed foods and maintaining a balanced diet.

Next For Xylitol

The biggest unanswered question is whether xylitol itself is responsible for the increased cardiovascular risk seen in the observational data.

Researchers are calling for additional studies, including research that can better determine whether reducing xylitol exposure changes cardiovascular outcomes.

The ESC says further investigation is warranted as the use of xylitol and other sugar alcohols continues to grow.

Evidence supports caution rather than panic.

Xylitol remains widely used, but the latest findings add to a growing scientific debate over whether replacing sugar with certain sugar alcohols necessarily produces the cardiovascular benefits consumers expect.
