Trump’s $90 Medicare Checks: Who Qualifies and When Will Seniors Get Paid?


President Donald Trump announced on Oct. 2 that the federal government would give over 20 million Medicare Part B enrollees a single payment of $90 in order to help them pay their premiums.

The money will be drawn from the Medicare Improvement Fund, which the White House has stated has been given $2 billion for the purpose of improving the Medicare fee-for-service program but hadn’t been used up hitherto.

On Truth Social, Trump said, “I am pleased to announce that my Administration will, immediately, begin sending ‘Checks’ of nearly $100 to over 20 MILLION wonderful Seniors to help pay for their Medicare Part B premiums, which we have already reduced by significant amounts.”

In 2026, the standard monthly premium for Medicare Part B is $202.90, and the annual deductible is $283, as stated by the Centers for Medicare & Medicaid Services; the $90 payment amounts to approximately 44% of the standard monthly premium.

Eligibility Rules Exclude Some Medicare Beneficiaries

The White House states that the vast majority of people enrolled in Medicare Part B are eligible for the payment, and those whose Part B premiums are already paid by Medicaid and individuals who pay an Income-Related Monthly Adjustment Amount, or IRMAA, are excluded.

IRMAA is an extra sum that Medicare beneficiaries with higher incomes pay in addition to the standard Part B premium, and the CMS states that the income-related adjustment applies to about 8 per cent of people who have Medicare Part B.

The White House also said that this payment represents the first instance in which an administration has used the Medicare Improvement Fund to directly reduce costs for Medicare beneficiaries.

When and How Payments Arrive

The White House says that most of the eligible beneficiaries should get the $90 payment by direct deposit in early October; the ones who don’t have direct deposit are expected to receive a paper check at the mailing address on file with Medicare.

People who are beneficiaries can telephone 1-800-MEDICARE (1-800-633-4227) to find out if they are eligible, and Medicare gives that number as its toll-free telephone number for enquiries concerning billing, claims and other Medicare-related issues.

The White House stated that people who wish to check the status of their payment can call the Social Security Administration on 1-800-772-1213.

The Medicare Improvement Fund

The Medicare Improvement Fund was set up by Congress in 2008, and at first it was intended to bring about improvements to the original Medicare fee-for-service program, which included Parts A and B.

The Congressional Budget Office stated that since the Medicare Improvement Fund was set up, lawmakers have altered the level of funding available to it 28 times, comprising 11 increases and 17 decreases. Furthermore, the CBO’s 2023 report indicated that the Centers for Medicare & Medicaid Services hadn’t spent any of the money from the fund since the fund was established.

CBO reported that $2.2 billion was initially made available to the fund in 2008. The amount has subsequently changed through legislation.

Trump framed the payment as part of a broader effort to reduce health care costs. “We are finally using this Fund, along with my Most Favored Nations Deals, to substantially LOWER costs for our Seniors,” Trump said.

The announcement comes ahead of the November 2026 midterm elections, when voters will decide control of the U.S. House and Senate.

The payment is a one-time benefit and does not change the standard Medicare Part B premium for 2026. CMS set the standard monthly Part B premium at $202.90 for the year.

Trump also distinguished the Medicare payment from a separate health care-related payment. He said the $90 senior payment is “separate from the equally substantial Refund ‘Checks’ we are sending to One Million Americans who were overcharged for their Unaffordable Care Act plans by Joe Biden.”
